Complex Medical Care Program – Spartan Grove Residence Spartan Grove Residence is licensed by the Ministry of Children and Youth Services to support nine (9) children from newborn to age 18+ with a variety of disabilities and medical conditions. This home is staffed 24 hours a day 7 days a week and is located in Greely, Ontario Canada. The children that reside there have high medical needs or chronic disabilities that require qualified and trained staff. Children may require night-time support for suctioning, chest care, oxygen support, G-tube feeding and seizure disorders. The home is wheelchair accessible, has a heated outdoor pool and has a Snoezelen room. Our team of dedicated and professional staff are also experienced in palliative care support. (see first photo below) Supported Living Program – Baypointe Crescent Baypointe Crescent supports four (4) young adults who have physical and developmental needs but are able to direct their own care and have a high level of family involvement. The focus of this home is family involvement, communication, social activities, and supporting the individuals with all activities of daily living. (see second photo below) Supported Living Program – Rocque Street The Supported Living Program at Rocque Street supports up to four (4) individuals in a duplex setting. The upper level of the home supports two (2) adults who have physical disabilities but are able to direct their own care needs to the live-in mentor. The lower level of the home has a separate entrance and allows for two (2) individuals live in a semi-independent manner through the assistance of the mentor. This program focuses on life-skills including but not limited to daily planning/organization, meal planning, budgeting, community awareness, cooking, communication, hygiene and social.
